Circulatory disturbances

 

 
AVK (Arterial closure disease)



The chronic AVK is found with about 11% of the men, is a very frequent illness, men fall ill five times more often than women. The sex differences disappear in the higher age groups.

The AVK is a name for all organic changes of the vessel diameter of the arteries (arteria) which lead to the Stenose (constriction), or Obliteration (closure). By the Stenose of the vessels it comes to the ischemia (blood circulation deficiency), hypoxemia, Blutverhaltung) and circulatory disturbances of the vessels dependent on care and organs. Fasteners of central arteries are mostly unique and limited to a segment. Peripheral fasteners are langstreckig and repeatedly.

Diagnosis

 

Artery fasteners of the limbs and the big supraaortischen branches can be judged without surgical aid.



Diagnosis:



1. Anamnesis: acute or slow beginning, was already gone through a cardiac infarction, kind and place of the complaints.

 2. Inspection: Skin pallor, skin redness, Ulcera (sore), necrosis (dead tissue)

3. Pulse state: Pounding, pulse controls of all accessible arteries

4. Functional test: Walking test, fist end practise

The first symptom of a circulatory disturbance (DBS) is often a strain failure (Fail by strain). In general the following signs are a cause of an advanced calcification:

Cold feeling, par-aesthetic Miss's feelings (Prickle, falling asleep of parts of the body), hyperkeratinization of the soles (increased corneal repulsion), alopecia, pallor with high-level storage, zyanotische Rötung with deep storage.

Typically for an arterial DBS (circulatory disturbance) wearing off rest pain is with deep storage of the part of the body. If strain pain exists, this wears off in rest again very fast. Also DBS are to be supposed with patients whose skin has lost her natural staining with which the skin is paper-thin, and with those one-sided hairs in the shank find

With the differential diagnosis the following factors / illnesses are to be separated:

DBS of the arms: neurovaskuläre shoulder girdle compression syndrome, Vasculitiden (vascular disease), Kollagenosen (connective tissue diseases)

DBS of the bones: Illnesses of the spine, inflammatory and degenerative illnesses of the supporting apparatus as well as neuro-logical illnesses


Risk factors



Exogenously: Smoking, cold, moisture, infectious diseases. Endogenously: Age, diabetes mellitus, Hyperlipidämie (fat increase in the blood), hypertension



Cause:



Risk factors are:

Blood pressure projection

Cigarette smoke inhalation

Hyperlipidämie (blood lipids projection)

Diabetes mellitus (diabetes)

Hyperurikämie (gout)

Overweight (indirectly)

With smokers it comes to peripheral fasteners, with hypertension the cerebral vessels are concerned. A diabetes quadruples the risk to fall ill with a limb closure.



 The following divisions are distinguished:



1. Forearm, shank type,

2. Femur type

3. Pelvic type

4. Shoulder type

In possibly half of the cases of illness with circulatory disturbances an arterial closure disease of the Obrerschenkeltyps is given.

- 30% of femur

- Peripheral type approx. 20%

The occurence of arterial diseases with the cooperation of several risk factors rises very strongly.

For example, strong smoking favours peripheral fasteners, while raised blood pressure develops as a risk factor in the central arteries (cerebral vessels) stronger. With the diabetes the risk quadruples itself to fall ill with a limb closure.

Stage division:

Stage I: Complaint freedom or untypical Miss's feeling

StadiumII: Strain pain, sucked. Claudicatio intermittens (shop-window disease)

Stage III: Rest pain

Stage IV: Gewebsuntergang (necrosis, sphacelation)


Therapy



General measures:



1-nicotine weaning, blood pressure treatment, diabetes adjustment, cholesterin and gout treatment, walking training

2-therapies: Elimination of the risk factors, systematic walking training, medicinal therapy, surgical plastic arteries, Bypass-opertion

3-in the later phases Drips with vasodilatation medicine, if necessary dose of anticoagulant.

4-homeopathies: The medicine which lead to treat the metabolism disorder.

5-oxygen therapies: for the improvement and care of the blood circulation in the areas with oxygen starvation.

6-acupunctures: to the removal of pains.
